This agreement outlines the terms and conditions for clients enrolling our Package 8888. By signing below, the client acknowledges and agrees to the following:

1. Payment Terms

• A deposit of RM 4444 (50% of the package price) is required to secure the booking.
• The deposit is non-refundable under all circumstances.

2. Booking & Rescheduling

• Clients may reschedule their procedure date once only.
• Any date changes must be requested at least 1 month in advance.
• Changes requested less than 1 month before the procedure will not be accepted.

3. Accommodation

• Accommodation is provided only from clinic-approved options.
• If the client prefers alternative accommodation, additional charges apply and must be borne by the client.

4. Package Pricing

• The package price is fixed.
• No discounts will be applied under any circumstances.

5. Transport Services

• If pick-up/drop-off service is required, the clinic must be informed at least 1 week prior to the procedure date.

6. Cancellation Policy

• In case of last-minute cancellations, sick leave, or emergencies, the deposit will not be refunded.

7. Medication

• The package includes DHT blockers and medications provided on the day of the procedure.
• Any additional medications must be purchased separately.

8. Consultations

• Package includes 12 personal consultations with the doctor.
• Additional consultations beyond the 12 sessions will be charged separately.

9. PRP Treatments

• Package includes 12 PRP sessions, with 2 used during the hair transplant (HT) procedure.
• The remaining 10 PRP sessions are available post-procedure.
• PRP sessions cannot be converted to other hair boosters or treatments, even with top-up payments.

10. Hair Wash Services

• Clients are entitled to unlimited hair wash services for up to one month from the HT date.
